Transcript: If the original transcripts (and the degree certificates from those institutions that do not indicate the awarding of degrees on the transcripts) are issued in a language other than English, you must have one official set in the original language delivered to the address provided below. A second official sealed set must be provided to a certified translator. The translator must provide a word-for-word translation of the transcript and notarize the translation. These documents must be put in a sealed envelope by the translator. The translator should then forward that sealed envelope of the original transcript and the notarized translation to the address provided below. Curriculum Vitae: A detailed resume indicating volunteer or paid experience relevant to social work. Please include educational history, professional experience (including teaching), professional and educational awards/distinctions, publications, and presentations. Personal Statement: A study plan (statement of intent) outlining the applicant’s educational goals and career expectations. The study plan must indicate the applicant’s intended area of specialization: Clinical Social Work Practice, Leadership in the Human Services, or International and Community Development. Recommendation Letter: Two reference letters, preferably one academic, and one professional. If an applicant is unable to provide an academic reference, a second professional reference can be submitted instead.
